What is Home Health Care?

Home healthcare is doctor-prescribed medical care and education provided intermittently in your home by a registered nurse, occupational therapist, physical therapist or other skilled medical professional as long as you remain home bound.

My doctor ordered home health care for me–now what?

Home healthcare is doctor-prescribed medical care and education provided intermittently in your home by a registered nurse, occupational therapist, physical therapist or other skilled medical professional (ie speech therapist and medical social worker) as long as you remain home bound. (You qualify as homebound if you leave your home with considerable and taxing effort to attend to your essential needs like doctors’ appointments, religious services, grocery shopping, etc.)

When is home health care used?

The following are examples of when home health care might be used:

  • Following an inpatient hospitalization, surgical procedure, rehabilitation, or stay at a skilled nursing facility- to help you transition back to Independence
  • When you have had a recent medication change- in order to monitor for side effects and to ensure a new medication is effective
  • When you experience a fall and/or overall decline in functioning- In order to help you regain independence through therapy or learning new skills to compensate for your condition
  • Following any change in your medical status that requires skilled care while you are homebound
How much does Home Health Care Cost?

Since home health care is prescribed by a physician, Medicare covers 100% of eligible home health care services

How long does Home Health Care Last?

During your initial home health care evaluation visit, your clinicians, along with your physician, will develop a plan of are for you during your time on service with Waypoint. Once those goals have been met, Waypoint Home Health Care will discharge you from service as long as you are safe in your home. (Most home health care patients are on service with us for 3- 8 weeks.)

Now that my doctor has ordered home health care, what happens next?

You can expect a call from WayPoint HHC within 24-48 hours of Waypoint receiving the home health care order from your doctor. Our scheduling coordinator will tell you what services have been ordered by your doctor and when you can expect your first visit.
