Waypoint’s team of Certified Dementia Specialists (CDS) possess advanced understanding of health, mobility and safety concerns that accompany changes in memory and cognition and will provide compassion, education and support as you and your loved one adjust to these ongoing changes. Our nurses are specially trained to optimize preserved abilities and reduce stress to all involved so that the highest quality of life is achieved.
Treatment
- Focused interventions for caregivers of patients whose psychological and behavioral issues interfere with daily living
- Teaching caregivers specific coping strategies
- Applying techniques to relieve stress and increase tolerance and cooperation
- Provide education on patient’s medications
- Help with preparing for and adjusting to ongoing changes
- Physical Therapy support to keep patients safe and physically active
- Support and Education to help maintain caregiver’s health
Program Goals
- Assess and maintain physical safety in the home so the patient can remain in place
- Manage comorbidities
- Ensure patient is taking all medications properly
- Avoid hosptalizations
- Maintain patient mental stimulation
- Minimize family conflict around patients who display uncooperative tendencies in ADLs, weight loss due to poor eating habits and agitated or aggressive behaviors
- Assist with management of caregiver stress
