Orthopedic Rehab
Often, rehabilitation at home following an orthopedic procedure is the best fit for a patient. Our program provides comprehensive hands-on physical therapy, occupational therapy and skilled nursing according to the protocols of the patient’s physician and will involve one-on-one therapy and education to return the patient’s range of motion, muscle specific strengthening, progressive mobility training and wound care. Waypoint Home Health Care’s team of experienced and caring therapists and nurses provide the highest quality of clinical care along with encouragement, compassion and motivation to help patients reach their goals safely and productively.
Program Goals
- Guide patient recovery around impairments due to issues related to muscles, bones, joints, and nerves
- Improve abilities, reduce symptoms and restore patient independence
- Reduce the risk of unplanned hospital visits
- Ensure patient is taking medications properly
- Ensure patient in-home safety prior to discharge from home health services

- Therapeutic joint range of motion exercises and training
- Strengthening exercises and training
- Functional mobility and transfer training
- Balance and gait training
- Focused adherence to a comprehensive physician-directed care plan to maximize outcomes and prevent complications and re-hospitalizations
- Medication management and education (including monitoring of blood coagulation therapies)
- Home safety risk assessment to reduce the risk of falls
- Concentrated focus during first two weeks of in-home rehabilitation
- Neuromuscular re-education
